Required Information

You must be physically present before the Commissioner of Oaths while the oath or declaration is administered in a way prescribed by law. It is important to bring a piece of existing identification with your signature on it, as the Commissioner of Oaths need to verify the authenticity of your identity and signature prior to you signing the document. Please complete all your documents prior to arriving at our office, EXCEPT for signatures and dates. It is the commissioner’s job to properly identify you and witness you signing any documents.
